On the Job Injury
The Campus Incident Form needs to be completed for every employee injury. Even if the employee does NOT want to seek outside medical treatment.
The 25.26 WC Employee Claim Packet needs to be filled out by the employee if they want to seek outside medical treatment. Employees can download and complete this fillable PDF. It must be emailed to [email protected] as soon as possible. (Prior to going to the urgent care facility.)
The Supervisor Investigation Report needs to be filled out by the injured employees supervisor and emailed to [email protected].
