You may fill out an application online on https://family.titank12.com/ or obtain a paper application from any of our school campuses, the Child Nutrition office, or mvisd website under Child Nutrition Department.

You may submit the application directly to the Child Nutrition office at 8449 FM 471 South, Castroville, TX 78009 or email Raquel.Cedillo@mvisd.org

Additionally, please remember to submit a new Special Diet request from your child's medical office if your child has medical dietary needs even if there was one submitted the previous school year.  If you would like to add any special charging restrictions on your child's account such as snack limits, please notify the Child Nutrition office at 830-931-2243 ext. 1154.
